That's why we expect any carrier interested in FTL Logistics to meet the same strict criteria and high standards of service that we demand of Fikes' owner operator fleet.

There are two ways to apply to our carrier program:

- A signed copy of our brokerage agreement;
- A copy of your operating authority certificate;
- A cargo insurance certificate for a minimum of $100,000 naming FTL Logistics, LLC as additionally insured*;
- An auto liability insurance certificate for a minimum of $1,000,000 naming FTL Logistics, LLC as additionally insured (for notification purposes only)*;
- Worker's Compensation insurance (if applicable);
- A complete W-9 form (that matches your 941 form); and
- Your D.O.T. rating (if applicable).
Please do not send a blank insurance certificate. Have your insurance company mail or email us a copy of the certificate: FTL Logistics, LLC, PO Box 703, Hope, AR 71802.
